last night we were running through velks and be it due to lag or some other reason we opened the chest before we got the update for talking to velk. this resulted in the loot being available but the mission not being completed, so no lockout, no coins etc.

I looked into this, and in Seeking the sorcerer the chest doesn't even spawn until after you talk to Velketor. And in both cases you get the lockout by opening the chest. I suppose it might be possible that some form of lag could cause a massive delay in the updating of the task so that the chest spawns and you can open it before the task updates, but that seems pretty unlikely. Not much I can do on my side to change that if it is why that is happening.
I have had this a couple times, not exactly common, but it does seem to be some sort of lag component.
