When you are using D pad to get a medical kit trying to save yourself from death. And you die anyway. The game opens up your death screen with the membership button lit up or the revenge button lit up. Not the respawn button. I have video and screen shots of this. The sticks don't trigger this bug, it is the D pad. Its a bug because you instantly go into the death screen with a BC option.
