I am not sure how widespread the bug or even if it is a bug, but there are times when I toss a grenade, I see it leave my hand, it skids down a hall loaded with people, I die, and the grenade never goes off. I respawn, my grenade count has not gone down. The game has not registered that I chucked a grenade into a room full of 40 people. Does this happen often, and what might be the cause of this, and is this working as intended?
Yes this is fairly normal, same with rockets.. can be very annoying when you chuck it in a room full of people just before you die
