I do think these drops are very random, regardless the place or mob, so I don't think making a database would help much. But here's what I remember: Blaze : can't remember, but it dropped in Antonica. Flashfreeze : Sabertooth looter in Thundering Steppes. Suffocating Breath : Giants in Thundering Steppes (Ott Stompgut are).
I understand that they are random thats why I want many people to post and that way we can create a instance of every place it has dropped because I have noticed patterns of drops from all creatures. I'm pretty sure theres a book pattern once taken a closer look at.

ive seen dark distortion drop off of skeles in stormhold and flashfreeze from giants in TS I found Suffocating Cloud in SH. After writing this, I found a putrid cloud by the skellies near Varsoons. I was so excited - it was the first useful adept that I ever found. Well, 2 weeks ago, Flashfreeze dropped from some Sabertooth loorters (in TS) while I was doing my Paladin's AQ5. Had Steal Breath Adept 1 drop last night off a Mighty Griffon while trying to get Bloodtalon to spawn Curse of Null dropped for me twice at the giants in north Nektulos Forest.

Flashfreeze from gnoll in TS. (Was the one stood on the bridge leading to the gnolls from the giants pit)... I know thats over precise - meh
