Previously, items could get their ornament bugged so that it thinks there is an ornament even when there isn't. There are still bugged items in the game however. For example, I have a helm and a weapon that are bugged. When you try to add an ornament, it gives yellow error text saying "The item already has an ornamentation applied. Unable to combine the item and the augment." (I'd show a screenshot of this, but these forums make that take way too much work.)
This can be fixed with an Ornamentation Stripping Tool purchased from an LDoN Magus (and other merchants). Combine the un-augged item in the tool and it will strip the current visual.
