   Encounter this bug in late May, June patch said it fixed it.
   
   Encountered it again last night.
   
   Live server to Live server.
   Character name had to be changed because original was unavailable.
   
   Support Ticket Number available on request.

   Yeah I reported this back in June after the update that was supposed to fix this issue.
