   I leveled to 95, and got decap 3. Then while I had raided, I died a few times and now am level 94, but Decapitation is no longer proccing.

   All AA's are like that. The final (decap 3) is level 95 required. You delevelled so have lost the ability to use that line. You need to relevel to 95 again.
