   I just signed up for 'All Access Membership' for the free lvl85 boost, and didn't get one...
   Is this boost certain server only or for all servers???

   It's for all live servers at least. I'm not 100% sure if TLPs that have heroics unlocked get it.
   
   I did the same as you and got All Access for a heroic. I found I had to restart my client a few times for it to show up, but it did eventually.
