A leaguemate was doing the daily where you have to enter an instance, clear the room of adds and kill the boss. He died while fighting the boss and it disappeared. He had to relog and head back to the instance and clear the room once again. (Happened on the sewer instance that has the same layout as the one used for the OMAC pre level 30 instance / T1 OMAC Duo. Adds were cyclops).
