The Reliquary of Tarew Marr for the Herald Quest only drops one in the chest and only the person who loots it gets quest credit. For a [Heroic] zone this should update everyone or be lore and drop one for each person.
We have not been able to reproduce this issue. Can you share additional reproduction steps / information (specific mobs, characters, etc) for us to follow? Is the issue still happening?
