After completing the full set of Copperleaf Chain Ornaments, I did not receive the achievement. On further inspection, the wrist ornament had disappeared from the present slot, but the wrist still shows the graphic, both in the inventory screen and on my character as though the ornament was equipped. When I try to insert another wrist augment, it states "That item already has an ornamentation applied. Unable to combine the item and augment". When I double click on the empty slot, it states "You are not holding an augment to insert" I do not know if it is related, but I had previously had issues with the Personas.
