Just started happening to me recently. I get countered, hit my breakout trinket (or Light Barrier), and I'm stuck for a couple of seconds...
Happens to me all the time in Wayward when fighting Grundy. }Them plant thingy will trap me... and even breaking out it will take 2 seconds, 3 seconds.... heck as much as 5 seconds before I finally break out. I dismissed it as being the lag causing it.
It's a bug.. It registers as a breakout (power cost, power recover) but you dont actually get freed. You should watch how fast your power bars goes down everytime you hear the breakout..
