Pulled this one off by accident and was able to reproduce it with a friend: If you are shot while spawning a vehicle - with the vehicle spawned (you see the green vehicle gui on the right) but you not yet being teleported into the driver seat - you die, but are stuck on the loading screen at 100% the next time you spawn. And yes, you are really spawned, I found out by being stuck in a restricted area (I only heard the sound of me being slaughtered by the damage field). Restarting the game is necessary, since you will always end up being stuck on 100% from then on. Probably very rare, but I thought I'd spread the word...
I have had this happen to me twice before, don't remember dying whilst pulling a vehicle either time though. Still a pain to relog to fix it.
