    Sounds like you have a failing graphics card. Not uncommon, some just don't hold up.

    appears that disabling speeedster and C states and Turbo helped a bit. But still crashing.

    You might want to try, if it hasn't been mentioned yet, upping your cpu voltage just a tiny bit and see if that helps. Also, do yourself a huge favour and get an AMD RX 480 when they come out. The game is not as CPU bound as others think and you will see at least a 20fps increase over your present card.
