This is a bug that happens not so frequently, but it has been happening lately a lot more than before. Of course it could be related to my hardware or software, could be my graphic's card overheating, but I really doubt that..
Ive never had it happen and none of my friends I play with have, while that doesn't mean its a you problem it does seem that way to me. check memory and CPU usage and make sure all your drivers are up to date/
