What exactly does accuracy do for you?

If you tried to stab someone in real life (please don't) a miss is a miss irrelevant of why it is a miss, either you stabbing in the wrong direction or the target avoiding the attack, in eq2 that is different, its treated as separate reasons, one as your failure, the other as the opponents success. Accuracy is for overcoming your failure to stab the other guy, strikethrough is for overcoming the opponents success at avoiding your stabbing.
So far the theory, now on the practical bit:
As others have said accuracy is broken, so don't bother with it.

hit/miss is a contested roll vs. your target and accuracy modifies you chance to hit your opponent the same way that defense modifies your targets chance to cause your hit to fail.
there are many different modifiers in the game. class of your attacker/target is a modifier, level of the attacker/target is a modifier, con (epic, heroic, solo, name) are modifiers.
everytime you swing the game takes into account all the modifiers and calculates if it's a hit or a miss, even if you have 100% accuracy you won't see 100% hit rates vs. every mob because it's just one of the modifiers.

best thing to do is get accuracy up until you see it no longer gives you a good benefit then step it back a pinch. also make sure that your hit rates are do to hit/miss and not avoidance(dodge/parry/block/riposte) as avoidance has nothing to do with accuracy or melee skill when calculating your hit chance.
