They're crafted, then added to an existing crafted or non-crafted item. They do a variety of things; add stat or resistance bonuses or skill bonuses or damage / powertap / lifetap procs. They come in Treasured, Legendary, and Fabled quality levels depending on what you had to blow up to make 'em. The raw materials are made by transmuters who blow up magic items in a loud and colorful but fortunately not dangerous way or else I wouldn't have any fingers left because it takes a LOT of blowing stuff up to level in transmuting. The raws are then combined into adornments. This is done either in the usual crafting process by the usual crafting classes, or, in a manner similar to but vastly more boring than regular crafting, by transmuters; it depends on which adornment you want to make.
