So I've been having a little look through ACT from the last few days. I'm averaging between zero and two procs of combination PER ZONE. Looking at logs from BoL, I was getting many many times that. I've checked the hit rate on my CA's and they're at or close to 100%. I have my filler CA's in banks of 3 for combination, and some of them have been used 15-20 times in a zone, with just 1 combination proc, or possibly none at all. Also it hits like a wet noodle. The tooltip value is about the same as 10 points in pressure point for me at 311k potency (about 65 million), yet pressure point is hitting roughly twice as hard.
