Seems as soon as you reforge anything on this charm, you loose the 25 CB over cap. The original vial, as well as the first upgraded model both keep the CB OC, not sure why the highest tier losses the OC with reforging.
Did a bug report on this roughly a week ago. I've only tried reforging it using CB and the ability modifier but, the item will keep its CB OC if you reforge using the ability modifier. Reforging the item using CB will result in losing its CB OC.
