Those "three" (it can be many more) never spawn if you cure all the AEs, but can be split up & killed however you like if you don't kill them as they appear.
You can do this mission with a merc tank and healer if you have great dps...if not it's best to have someone dedicated to doing nothing but curing so you don't get the adds.
Like others said. Follow the mechanics of the mission and you wont get any adds. Cure when u get the dot, kite the cloud and don't run it over others. not really that hard.
And unless its been fixed after both clouds up. Evac and repeat and the clouds don't respawn making the achievement easy.
