Critical success on uncommon conversion caused one agent to be saved, but did not pop up a reward window, did not reward the rare agent, did not repopulate the uncommon conversion quest, does not exist in the quest rewards window, and does not appear in the overseer quest list. Finishing a different quest afterwards DID result in a reward, but did not spawn the missing reward. Logging out and back in did not fix this issue.
