I don't want to wade into a debate about semantics. It's probably 'to-may-to' or 'to-mah-to' kind of distinction.
So the term 'chase loot' has been used interchangeably in the past to describe ultra rares, global rares, and zonewide rares.

Are they being removed from players as well as mobs?

The upgraded Sewers and Lavastorm loot was a mistake. It was corrected. Expect further corrections.
Unfortunate, out of era loot would have been cool and more appealing just for the mischief of it all.
I imagine what happened is while they for example removed most of the named rares that spawn in lavastorm that were introduced in DoN they did not remove the loot from the tables, so its dropping from other mobs of similar levels.
This is TBD, but not right now.
Might be worth setting a precident now by exchanging those items for something comperable in era, just in case (until) this happens again. If they are left in game then something more overpowered may accidentally drop later and people will point back at this moment and say they should be able to keep them.
Or you could be d*cks and just exchange the items for Sword of Truth. X-D
Stuff is pretty jacked now after resets. On Mischief can't target mobs and the translocators aren't present in Butcherblock Mountains. Translocator Fithop eventually popped, but then suddenly died - "Translocator Fithop died."
Is this why all the merchants in Feerott have nothing for sale, even their fixed item list?
oh yeah, that, too. Merchants in reset zones have none of their normal inventory. Confirmed in Greater Fay and South Kaladim.
Yep, translocators keep occasionally spawning and insta-dying.
