Dodge isn't a 100% success, the skill is used, not necessarily successfully, whenever the previous avoidance's on the list fail. I've received skill ups on my druids dodge skill when he got killed without successfully dodging. I don't know the exact mechanics for a successful dodge, I'm assuming it's weapon skill vs dodge or something equally as simple.
