The achievement for Gandak's Ill-Fated Trip is currently hidden. It's counted in the total possible achievements despite this. It can still be completed, and achievements points and total completed increase with its completion. It's just not currently visible, and clicking the View Mine button results in the error "You cannot view that achievement."
