This is an issue that has been going on for how long now, can something please be done about it? It happens virtually on a daily basis, some days 2 - 3 times. Today I was grouped for E3 Escape, supercharge ready to go, I went through a tele-porter and boom, crash with the usual PS Error

    Way too often.

If we've all done the "report problem" when this happens, you'd think that there would be more than enough data to identify where this problem occurs and why.

Sony says the fix is to reinstall the latest PS4 system software and I believe reinitialize your PS4. I don't buy that. General crashes like this tend to be things like an "array out-of-bounds" error or other memory leak. One site said something about corrupted local cache files producing this error too.

This is crap! I turn ps4 off out of anger if was in an operational instance, this needs addressing.

Is there any pattern about where or when or which teleporter causes it?

Haven't noticed a specific pattern, I just prepare myself mentally that it wIll happen any day, any time, any place. It is however rubbish when it happens during an instance or when grouped for one

    Wireless or wired connection?

I encounter this a lot, but as I'm entering/exiting police stations, or warping to my base/HQ. I'm a PS4 player using wireless connection. (EUPS server).

I encounter this a lot with no rhyme or reason. I've had it happen leaving/entering warehouses.. teleporting to the headquarters or my base. I have two ps4s in my house, one wired and one wireless, happens on both equally.
